Services
Thank you for your interest
in our clinic. We welcome the opportunity to provide services to
you. If you would like to schedule an appointment for an evaluation,
please email us or (907) 562-4550.
In order to schedule an appointment, the office will request that you complete
a series of forms. All forms can be downloaded from your computer.
Below is a list of required forms which must be received before evaluation
time. There is also a list of additional forms which may be required.

What to expect:
First:
We schedule a 30-45 minute
hearing evaluation appointment. At that appointment, we do a complete
diagnostic evaluation looking at hearing levels, health and function of
the ear and whether additional referrals are needed. If the patient
is interested and has hearing loss that would benefit from a hearing device,
that can be discussed in depth. There will be time for questions
to be answered. If the patient is ready to purchase a hearing
aid, it can be ordered at that time. If the patient is not
ready to purchase a hearing aid at this time, they can return at a later
time for a hearing aid consultation/evaluation.
Second:
The patient returns for
a 45-60 minute appointment for hearing aid fitting or to discuss ordering
a hearing device. The hearing aid fitting reviews how to use the
device, including care, warranty, and troubleshooting. The hearing
device is specially programmed for the patient’s hearing loss, listening
environments and ear anatomy. The patient pays for a
one time hearing aid fitting but may come back for complimentary adjustments
and cleanings during the first year.
The patient is required to
have medical clearance or sign a medical waiver prior to being fitted with
a hearing aid.

All forms are also available in their original Microsoft Word or Excell
format which can be filled out electronically and emailed back to us at
mary@aktherapedics.com.
In this case your signature will be obtained when you arrive at the office.
Forms which have been altered in any way other than with requested information
will not be accepted.
